Fresh Alliance Reporting

OFB Partner Agencies who receive retail donations through the Fresh Alliance program are required to report the pounds of product received on a monthly basis. Reports are always due by the 5th of the month after donations are received. You will receive a reminder email approximately 5 business days before the reporting deadline.

Please save a copy of the Fresh Alliance Reporting Template with Instructions (linked below). Completed reports should be emailed to **in Excel format**, not PDF please. Alternatively, you may share the report as a Google Sheets file with
