Welcome to the Agency Contact info Center. Here you can update your contact information using one of the designated forms below. 

Agencies in the Statewide Network please use the Statewide Network Agencies Information Update form. Agencies under Multnomah, Washington, Clackamas Services, Southeast Oregon Services, Tillamook County Services, and Columbia Gorge please use the Agency Information Update Request form. 

Please update this form proactively and verify the accuracy of your existing contact info before submitting changes. To review your agency's current contact info follow these steps: 

  1. Click "Order Online" and log in
  2. Click "My Agency" in the top right-hand corner of the screen.
  3. Review the "Contacts" tab located under your agency name.

Agencies in the Multnomah, Washington, and Clackamas Counties, and agencies under the following OFB branches; Southeast Oregon Services, Tillamook County Services, and Columbia Gorge use the Agency Information Request Form.  (If not applicable to your agency use the form below)

Please complete this form to let us know about any pertinent changes to your Agency. (Hours, Staff, Address etc.) These changes will be made on Food Finder and Primarius 2 accordingly. 

Please allow one business day for changes to be reflected.
