As an Oregon Food Bank Partner Agency, it is crucial that we have the most accurate infomation for your site. We have just added a new form to our website with step-by-step instructions for updating agency contact information. It is your responsibility to let us know if there are any changes to your staff, hours, address, or programming. We recommend that you look up your agency on Food Finder to ensure that we have the most up-to-date location and hours information. If the information listed is not accurate please fill out this form, so that we may adjust your agency information. Clients may receive inaccurate information unless our team is made aware of these important changes.
Staffing changes should also be communicated to OFB using this form. We need to have the most up to date staffing information for a variety of important reasons. One of which is that we may need new agreements signed if one of your signers is no longer working at your agency. You can indicate who is leaving AND a replacement on the form. Recall contact information is another crucial reason why OFB must be made aware of staffing changes. If there is a product recall and we don't have the right person to contact this could greatly impact the safety of your clients.
Additionally, we have added a new form to portals where Partner Agencies can request food safety supplies and posters! Please now request supplies using this form instead of sending requests via email.
