Even though we offer paper forms, we still recommend doing live intake when possible. Live intake allows users to:

  • Gather more complete and accurate information.
  • Save money and time spent on printing, paper, and document disposal.
  • Connect more with clients and provide custom referrals.

We also understand that sometimes live intake is not possible. For example, if the environment does not allow for a private interaction or if there is a language barrier. If this is your situation, a paper intake form may work well as an alternative. 

Our paper intake forms are offered in 16 different languages and can be downloaded to print from the links below. There are 3 different kinds of forms: TEFAP Intake Form, Non-TEFAP Intake Form, and Anonymous Intake Form. 

Each agency's forms must include a group of core questions. Regional Food Banks have the option to add up to 3 optional questions, so each Regional Food Bank's forms may look slightly different from other Food Banks'. Please click on your Regional Food Bank's name below to view your intake forms.

Tips when using paper intake forms:

  • Print double-sided to save paper.
  • Make sure forms are checked for legibility and completeness when turned in when the neighbor is still present. 
  • A SIMC card can be given to the neighbor once the paper form is turned in. Write the number listed on the card in the space on the top of the paper form that reads "Barcode #." The person who enters the information from the form in SIMC will link that card number to the neighbor's profile in SIMC (see "SIMC Cards" section of the Basic Training for more information on using SIMC cards). 
  • Use the English form to translate other languages by comparing the number or letter indicated to the side of each response.

**REQUIRED**
If any paper forms are taken offsite for any reason, their carrier is responsible for keeping the documents secure at all times. This means keeping them in a privacy envelope/folder when in transit, in a locked cabinet if storing, and shredding or disposing of in a secure manner once they are entered into SIMC. Client confidentiality must be protected at all times. This includes not allowing anyone to view them who has not signed the confidentially agreements.
