To help prevent the spread of Coronavirus at food assistance sites, the Oregon Department of Human Services (DHS) and the USDA Food & Nutrition Service (FNS) have waived the requirement for clients to sign the intake form. Instead, clients may tell an intake volunteer or staff member their name, address, and the number of people in their family, and verbally declare that they meet the eligibility criteria for TEFAP. In lieu of a signature, intake personnel will write “verbal” on the signature line of the intake form.
Instructions for agencies using paper intake forms (Form 4)
Ensure that 2020 TEFAP income guideline posters in all necessary languages are clearly visible from the point of intake
Intake should be set up so that information can be provided confidentially while also following current guidelines for social distancing.
Intake person should ask the client their name, city or zip code or an “H” for houseless (full address not required) and number of people in their family, and record it on Form 4.
Intake person should direct the client’s attention to the TEFAP income guideline posters and ask, “do you meet these eligibility requirements?”
If the client affirms their eligibility, write “verbal” on the signature line on Form 4.
If the client brings an Authorized Representative Form, they may verbally declare eligibility for the person named on the form. You should look at the form, but do not touch it or take it from the client. Clients should take the form home with them.
